The Patrolmen’s Benevolent Association and Davis Vision announce the following enhancement to your Vision Care Plan, effective September 1, 2004: Beginning September 1, 2004, members will no longer be required to obtain a certificate from the Funds Office, prior to receiving benefits. Your Davis Vision network provider will obtain the necessary authorization; no certificate or claim forms are required for services received at a network provider. Simply choose a network provider and make an appointment. Identify yourself as a PBA member and give the office your Identification Number (your Social Security Number). The provider will verify coverage for you. If you have any questions regarding the vision plan, visit the PBA website at www.nycpba.org (click on the “Benefits” icon, then click on “Davis Vision”; this will connect you to the Davis Vision website). Once you log in, you can verify your eligibility, locate participating providers in your area, view benefit information, and access your Personal Frame Collection. From the Davis Vision homepage, click on the “Member Login” icon. At this point you will need to enter your member ID (your social security number) and password (the first 5 letters of your last name) and click the “Submit” icon. This will bring you to your personalized menu screen which lists all that is available to you as a PBA member. Alternatively, you may contact Davis Vision at 1 (800) 999-5431. Beginning September 1, 2004, the Funds Office will no longer process out of network vision claims. If you receive services outside of the network on or after September 1, 2004, your claim for reimbursement should be sent directly to Davis Vision. The address to submit claims is P.O. Box 1525, Latham, NY 12110. Out of network claim forms are available at the Funds Office, by calling Davis Vision, or via the PBA/Davis Vision link at www.nycpba.org. All other Vision Care Plan benefits will remain the same.
