Re: Extension to File FICA LOD Consent Forms

We are informed by the Office of Labor Relations that approximately 3,000 of our eligible members have not submitted consent forms to claim their FICA Line of Duty refunds. The City has provided us with a list of tax registry numbers for all members who were out Line of Duty for less than six months during the years 1989-2005 (see links at right), who have not submitted consent forms.

We urge all members on the list to file the necessary forms no later than November 30, 2007. If you are on the list, but have not received a consent form in the mail or have misplaced it, contact the Office of Payroll Administration’s FICA Refund Claim Unit by e-mail at Lodi_FICARefund@payroll.nyc.gov or by telephone at (212) 669-2333, (212) 669-4133, or (212) 669-3669.
