The PBA General Counsel's Office has recently assumed the responsibility
of preparing simple wills and related documents for active and
retired members and their spouses.
Wills will be prepared subject to verification
of a member's eligibility by the PBA Funds Office (212-349-7560).
The plan covers simple wills for members domiciled in New York
State. Certain considerations may warrant that you go outside
the plan to secure an attorney, including: an out of state domicile,
the amount of assets involved in your estate, the nature of
your assets, or the complexity of family relationships having
claims on the estate. Similarly, we will not provide any form
of tax or estate planning in connection with the preparation
of your simple will. Anyone desiring such services should retain
their own attorney.
In order to prepare your will, we need to obtain
certain information about you and/or your family and assets.
We ask that you provide this information by completing the
Confidential Will Interview
Form. It is important that you complete this form
accurately as the information will be used to draft your will.
If you are requesting a will for your spouse, please have your
spouse print and complete an additional form.
We have also linked Living
Will, Health
Care Proxy and Power
of Attorney Forms, and Frequently
Asked Questions regarding these documents. If you
wish to make effective any or all of these documents, kindly
provide the information requested on the forms and send them
back to us. All completed form(s) should be sent to:

Attention: David M. Nicholson
If you have any questions about how to complete
any of these forms, contact David Nicholson, Assistant General

review. Once you have had an opportunity to review them and
confirm the accuracy of the information contained therein, you
should contact the General Counsel's Office to schedule an appointment
to finalize and execute the documents.
